Extended story: Override cluster in a card book

Reviewers found that seventeen files cited “relationship manager knowledge of income” without an employer call log, and nine carried dual approvals from the same acting supervisor using shared credentials.

The findings memo did not recommend shutting the channel. It recommended freezing that override reason code, restoring dual-control on temporary supervisor IDs, and sampling twenty new files two weeks later. The follow-up sample showed three remaining soft rationales — enough for the credit committee to keep the freeze another month.
